Acknowledgment and Assumption of Risk: I as the parent or legal guardian of the minor child named above, hereby acknowledge that participation in the Simonds Golf Academy Open Day involves certain inherent risks and hazards, including, but not limited to, physical injury, equipment malfunction, and the natural conditions of the golf environment. I understand that these risks and hazards may result in serious injury or harm to my child.

I willingly assume all risks associated with my child's participation in this event, whether known or unknown, including those that may arise from the actions, omissions, or negligence of other participants, volunteers, or staff.

Waiver and Release: In consideration of my child's participation in the Simonds Golf Academy Open Day, I, on behalf of myself, my child, and our respective heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless Simonds Golf Academy as well as Mass Golf Lab, its officers, directors, employees, volunteers, and agents (collectively, the "Released Parties") from any and all claims, liabilities, demands, causes of action, damages, losses, or expenses of any kind, including, but not limited to, personal injury, property damage, or wrongful death, arising out of or in connection with my child's participation in the event.

I understand that this waiver and release extends to claims which I do not know or do not expect to exist in my favor at the time of signing this waiver and release.

 

Medical Consent: In the event of a medical emergency, I authorize the staff and volunteers of Simonds Golf Academy to seek and obtain appropriate medical treatment for my child. I understand that I am responsible for any and all medical expenses incurred as a result of any injury or illness sustained by my child during the event.

 

Photographic Release: I grant permission to Simonds Golf Academy to take and use photographs and/or video recordings of my child during the event for promotional, educational, or other lawful purposes. I understand that these images and/or recordings may be used in various media, including, but not limited to, brochures, websites, and social media platforms, without compensation to me or my child.

 

Acknowledgment of Understanding: I have read and fully understand this Waiver and Release of Liability. I acknowledge that I am agreeing to this document freely and voluntarily and intend my acknowledgement to be a complete and unconditional release of all liability to the greatest extent allowed by Massachusetts law.
